Evaluate screening utility beyond a detectable signal.
A proposed methylation test needs a clearly defined population and intended use. Institutions should request assay-specific evidence and examine the complete pathway after a positive, negative or unresolved result. Technical performance and benefit in an asymptomatic screening programme are separate questions.
NCI explains that the effectiveness of multi-cancer detection tests for screening people without symptoms remains uncertain and requires evaluation in randomised clinical trials.

Questions for the programme brief
For which population and use?
Define age, risk, symptoms and whether the proposal concerns screening, diagnostic investigation or research.
Which evidence is available?
Request prospective data for the proposed assay and population, including false results, stage distribution and follow-up completeness.
What follows a positive result?
Review diagnostic work-up, referral capacity, unresolved findings and how harms and outcomes would be evaluated.
What to prepare for review
- Intended population and assay specification
- Assay-specific validation and clinical evidence
- Follow-up and outcome-evaluation plan
